Terms and Conditions:

Please note your team CANNOT play in the tourney if you do not provide a signed waiver form prior to the tourney day. You will be contacted by a representative of Fusion Physical Therapy. Fusion will be collecting player insurance information to make it easier to perform services on the day of the tournament. You may be asked to move to a bracket that is not the one you signed up for if demand warrants it.
